If you do not see your insurance network, we will be happy to verify your physical therapy benefit coverage for your treatment at Carefree Physical Therapy.

This is a medical background and insurance question form. You can download, print and complete it prior to your first visit to save time. Just bring it with you on your next visit.



List all prescribed and over the counter medications you are currently taking, and list any allergies or sensitivities to medication you may have.


Important definitions you should know about your insurance

DEDUCTIBLE:  A deductible is the amount you pay for health care services before your health insurance begins to pay providers.

COINSURANCE:  Is your share of the costs of a health care service.  It is usually figured as a percentage of the amount allowed by your insurance company for medical services rendered.  You become responsible for coinsurance after you’ve paid your plan’s deductible.

COPAY:  A copay is a fixed amount you pay for a health care service, at the time of your service.  The amount can vary by the type of service you receive and your individual health plan.
